New Student Registration

New student to the District

If you are registering your student to the district, please provide the School Nurse with an updated copy of their immunization record. Your student may require additional vaccinations if they are from a different state or country per NJ state regulations.

Please have the doctor complete the Universal Healthcare Form and submit it to the School Nurse.

Please complete a Health History Questionnaire and alert the School Nurse of any medical concerns.
